I didn't watch Idol School so I'm not sure, but I remember in the immediate aftermath a lot of knetz were complaining/confused about why Herin (ex smrookies) was eliminated so maybe her? She and Nayeon were both removed before the finale maybe to make the final result seem less suspicious

The contestants and i believe there was some charge to the viewers to vote as well. It was a small fee but they voted multiple times that would add up throughout the season.

Probably rigged-out for the same reason Kaeun was rigged-out for which was for looking "too mature". Proves that you have to be a "youthful" kind of beautiful as opposed to just beautiful cause if Haein and Kaeun looked like they could've been 18, Haein would've remained #1 and Kaeun would've been in IZ*ONE.

This is a good point. Like, in Iz*one for example Eunbi is literally only one year younger than Kaeun but she looked a bit more youthful so nobody was calling her "too old for the group" or at least I don't remember most people dragging her for her age like they did with Kaeun.

I think exposure is the key here. Its like with Kim Dani who was treated as old despite being on the younger side.Or Heo Chanmi being seen as ancient due to her connection to SNSD. Noone cared she was basically same age as Eunbi during Produce 48. Same to Somi being treated as overused goods and old when she debuted solo when in reality she was only 18yo. 

Kaeun was long time debuted as well while Haein lost her freshness to P101.
