Some people have a naturally small waist due to genetics. For example, my waist naturally measures 22 inches and I'm a tiny person all around (I'm only 5 ft/152cm tall and having a 22 inch waist at 5 ft/152cm isn't unusual or unhealthy). The late Goo Hara who was known for her ant waist also attributed her small waist to her genes.
