It's his hand allergy, and some of his other medical complications.

This is also the reason why Cai Xukun takes alot of medicinal herbs and pills. KeepRunning(Running Man in Korea)

is filmed outside in an area that can trigger his allergies. There's also a rumour that his absence is due to his ongoing court hearing

It will probably air a month or so. From what I remember about keep running it takes awhile for them to edit the episode.
