The form itself is to actually submit the final badge. Before that though, you have to determine how many people are going to get the badge once it comes out and make the list of confirmed purchasers. (not sure if this is a requirement) For example, Shannon Williams didn't have a badge last year and so someone with a CAF decided she was going to use her form to make one for her. She went to the Shannon Williams thread and asked who'd get he badge if it came out. After that, you make a Badge Voting thread so either those confirmed users or anyone else can give suggestions as to the pictures they want for the badge. You can ask Awards Team to make them into badges or make them yourself/other users Then you start voting until you are left with one badge. And with that, you submit the final form with the list of people who will get the badge and what the final badge is going to be. Correct me if I'm wrong!
