I dont know why but seeing Myung Ju at the hospital a few episodes earlier I had the feeling she would kill either Kim Jiwon or Jin Goo. I've had a bad feeling since the beginning, even tough there are things that give me hope. I like to think I've made my peace with it, but if it truly happens I'll probably cry for a whole week. I think she's just making good of the looming threat Mu Yeon had the entire run of the drama - that Si Jin is always in danger and one day, he will come back dead. The scriptwriter just want to manifest that idea and make it more real, and also adding some adrenaline to the Seoul scenes, because it can get boring if we are so used to the action-packed Urk.
