Miryo and Narsha got a lot done, JeA got pretty much done too, and Ga-In did her nose and maybe her jaw too, but not sure.

 

There's not much info about T-ara's surgery but Boram and Soyeon did the most. Soyeon got her nose done, and I think she might've gotten something else done to her face too long ago.

Boram... nose and something else.

 

Jiyeon might have had some work on her nose but I'm not sure.

Qri... I don't know, but might only be eyelids.

Eunjung I think is natural but might have done eyelid surgery.

Same with Hyomin.

 

I think,

Boram, double eyelid, jaw, fat graft(during Bunnystyle) and maybe nose

Qri, eye lid and maybe also her forehead and chin

Soyeon, had double eye lid and a nose job

Eunjung, I think only eye lid

Hyomin, eyelid and a minor nose job, she might've done something to her jaw or chin too i'm not sure

Jiyeon also her eyelids, also maybe cut those eye fold/epicanthic fold
