Kiko has collaborated with Opening Ceremony twice, her line is called Kiko Mizuhara for Opening Ceremony. In that photo Beyonce is wearing both a top and pants from Kiko's first collection, I think. Victoria and Sulli from f(x) also wore pieces of her collection in the Rum Pum Pum Pum music video.

It's simply a stage name. Her real name is Audrie Noriko Daniel. If you go back to when she was a Seventeen model (that was around 2005 iirc), she was credited as Noriko Daniel then. She said once that Kiko comes from a fairytale, as for Mizuhara idk.

I don't know if she actively smokes, but she was once kicked out of a shoot because of her smoking lol (she was underage at the time, 17 iirc).
